Sun Spots and Photodynamic Therapy

Filler, filler on the wall, who’s the greatest of them all?
The availability of a growing number of skin fillers have afforded experienced physicians with safe and effective tools for natural rejuvenation. Until recently, dermatologists and plastic surgeons endorsed a “tighter is younger” philosophy. This often did not produce a youthful look at all. One need look no further than some older well known celebrities who look almost emaciated with their tight, drawn facial skin overlying the sunken hollows of the cheeks. A little scary, isn’t it?

These less than ideal outcomes were due to two main problems. The first was a philosophical one. There was too much emphasis on tight and to little on contour. The second was the lack of natural and lasting fillers that can help restore a more youthful contour to the face.

The modern approach to facial rejuvenation fully appreciates the natural contours of the more youthful face. The young face has an “apple like” shape, with full cheeks tapering down to the chin area. As the face ages, there is loss of that upper cheek fullness and the face often seems deflated. There is often wrinkling around the eyes and fullness in the lower face. Folds and wrinkles around the mouth and jaw line are common as are creases along the upper and lower lip. Sometimes a slight downturn at the corners of the mouth gives an unintended sad or “sour” appearance. Enough of the negative, let’s talk about some good solutions.

The good news is that many of these “signs of aging” can be improved and reversed. Well chosen fillers in skilled hands can immediately bring back a more youthful appearance. Gently placing appropriate skin filler into the upper part of the cheeks can restore a more youthful facial appearing and partially “lift” some of the sagging of the lower face. Under eye hollows can be filled reducing the tired appearance that often accompanies the hollows. Nasal folds, marionette lines, and jaw line jowls can be softened. Creases around the mouth can be reduced and lost lip volume can be restored.

The Look You Want
    Contrast these following reactions:

  1. Wow, you look great; have you been away on vacation, are you working out, did you find a magic diet, maybe a new cream? The point here is that they know immediately that you look great but they cannot put their finger on why.

    Now, in contrast:
  2. Yep, she’s had work! I wonder who she is trying to look like.
I believe we all strive for that first reaction. We want to look more youthful, healthy, and vibrant. Most of all, we want to retain our identity and look like us. This is philosophically what we strive for with all out rejuvenation procedures. The right procedure, the right product, delivered by the right hands, for the right person.

MEDICAL NEWS AND CONTROVERSIES (What’s New in the News)
Vitamin D and Sunshine

Is unprotected sun exposure a good way to get adequate vitamin D?

NO. There is accumulating evidence that adequate levels of vitamin D are important for healthy immune functioning and may play a preventative role in some cancers. However, unprotected sun exposure is not a wise method to increase your vitamin D. Unprotected sun exposure is a good way to prematurely age the skin giving you wrinkles, brown spots, rough leathery texture, and a greater chance of developing skin cancer. Adequate Vitamin D can be obtained from a diet rich in vitamin D foods and if appropriate, vitamin D supplementation. There is no evidence that “sunshine vitamin D” is any different or better for your body than dietary and supplement vitamin D.

Caffeine

Yes, it is good for your skin.

Caffeine has been vilified by the medical community and media in recent years. The good new is that moderate caffeine consumption (1-2 cups of coffee or tea) appears to offer benefits including better memory and enhanced exercise performance. Even more important, topical application of caffeine appears to have protective and anti-aging effects for the skin. Products such as Replenix cream combine green tea polyphenols, soy, hyaluronic acid, and caffeine for antioxidant protection, anti-inflammatory effects, and rejuvenation of the skin.

Does Stress Lead to a “Nervous Breakdown” of the Skin?

Well, apparently it can! A recent study has added additional credence to the observation that the skin behaves more poorly when we experience greater levels of stress. The study demonstrated that the skin barrier becomes impaired allowing more irritants, allergens, and even infectious agents to penetrate the skin. Moreover, proteins that comprise the skins natural immune defense against infection were diminished.

So, don’t be a victim. Do something to reduce your stress! Massage, facials, aerobic exercise, meditation, rhythmic breathing, Yoga, Tai Chi, and guided imagery programs all have been shown to decrease stress and improve skin functioning for many people. Consider our CD programs for acne and psoriasis. Patients who have Botox and other rejuvenation procedures routinely report decreased stress levels.

See Spot Run: Do brown spots affect how we are viewed by others?

Apparently YES.

Brown may work well for UPS but apparently not when it appears as spots on the skin. A recent study found that individuals with visible age spots on the skin are judged as much older (up to 10 years) then those who are free of these discolorations. Lasers, light therapies, fruit acid peels, microdermabrasion all can be effective in reducing or eliminating this pigmentation from the face, chest, arms, and hands.
Rejuvenation therapies at YDA:
The latest in gentle treatments to restore that youthful look and glow
  • What's the Latest in Skin Rejuvenation Using Fractional Technology?
Are you interested in facial rejuvenation, wrinkle reduction and tightening of the skin on your face, neck and chest? The new Palomar Fractionated resurfacing anti-aging laser can do all that and more using state-of-art fractional laser technology. It is the one of the latest innovations for treating wrinkles, skin laxity and sun damaged skin. This revolutionary technology uses numerous microbeams of laser energy to stimulate the collagen and tighten the skin. Fractionated resurfacing reduces wrinkles, improves acne scars, and improves overall skin texture. The treatment is fast - taking only about 15 minutes for the whole face. Typically 4-6 treatments spaced 2 to 3 weeks apart are required for optimal results, but noticeable results are seen even after the first treatment. Some short lived redness and swelling of the treated area is usually seen. It most often subsides in a day or two and is easily covered by makeup.

  • Photo facial-IPL:
Photofacials using the state-of-the art Lux-G IPL utilize gentle pulsed light treatments to rejuvenate and improve the appearance of sun-damaged skin. Brown spots, unwanted blood vessels, and background redness are reduced. If you have a tendency for facial blushing and flushing, this is often improved as well. The skin looks smoother, tighter, brighter and clearer. Intense Pulsed Light (IPL) is used in a series of 4-6 treatments, depending on the patient, at 3 week intervals. It can be combined with microdermabrasion and other rejuvenation techniques. Photofacials have been shown to improve the luminosity of the skin, restoring a more youthful glow.

  • Botox: The Solution For That Stressed And Distressed Look
Among the many amazing uses and beneficial effects of Botox, we find that it produces a relaxed, rested, and substantially more youthful appearance. Botox treatment can eliminate the distressed and angry appearance that results from the creases and furrows produced by tense facial muscular contractions.

  • Fillers: Radiesse, Restylane, and Juvederm — A Lunch Time Lift
These calcium hydroxyappetitie and hyaluronic acid skin fillers allow for wonderful and immediate correction of wrinkles and sunken areas. Our patients are delighted with the natural-looking results in both the lower and upper areas of the face. These products give a more youthful, rested and attractive appearance. The process of gently restoring volume in the upper face actually lifts the lower face resulting in what has become known as a “non-surgical facelift.” These substantial and gratifying results are usually achieved with minimal or no complications-and many facial areas can be treated. The entire procedure takes about fifteen minutes and does not disrupt our patients' lives. The convenience of these “dermal fillers” has made them a mainstay for patients with busy schedules who are looking for an alternative, as well as an adjunct to, surgical options. When used in combination with Botox, it is amazing how much more vibrant and youthful you can look.

  • Laser therapy
Acne is one of the most common dermatologic disorders affecting about 80 per cent of the population. We are seeing a dramatic increase in adult acne, as well as teen acne, which can be very frustrating and emotionally devastating. Our adult female patients suffering from acne breakouts often complain that dealing with acne together with age and sun related lines and wrinkles seems especially unfair. The big news in acne treatment is Photodynamic Therapy. A compound is applied to the skin then activated using light either from a blue light or a laser. Using this combination approach, we can produce a reduction in sebaceous gland activity and improvement of acne lesions even faster than with oral antibiotics. We are now able to apply some of the same techniques used for wrinkle reduction and photo-rejuvenation to acne scars and active acne with very impressive results. Patients can benefit from less frequent break outs and an overall improvement in skin texture.
Acne and Rosacea
"Epidemic" is the word frequently used to describe the growing population of people struggling with acne and rosacea. The visible and often tender skin lesions can cause both physical and emotional discomfort. Our rapidly expanding array of treatments can usually provide sufferers with effective treatments for active skin blemishes as well as the skin coloration and scars that are often left behind.

Dr. Richard Fried’s book Healing Adult Acne: Your Guide to Clear Skin and Self-Confidence can be a useful tool to help you to choose the best treatments and understand how they work. The book also provides methods to identify acne triggers as well as self-help and stress management strategies.
